Any
team who rates their opponent a ‘-1’ must complete
this form by 5pm the day after your game
detailing the reason(s) for the poor rating. If you choose
not to file the report your opponent’s rating will be
changed to a ‘0’.
In order for the CSSC to effectively
follow-up with your opponent you must answer the following
questions, in addition to any other comments you may feel
are applicable. Your report will, in most cases, be sent directly
to your opponent so it is important the comments are constructive
and laid out in a positive way.
-What were the names and/or descriptions
of any players who were causing problems?
-What exactly did they do or say
to cause your team issues?
-How did you address the issue at the facility?
If the issue was not addressed at the facility, it may be
difficult for the CSSC to follow up. As outlined in the online
meeting, captains must make an effort to resolve any conflicts
at the facility. If the issue is addressed, and the team does
not improve, the CSSC will then follow up with the team.
-Did your opponent make the required effort to change?
-What could you/your team or your opponent do next time to
keep the game from deteriorating to a below average?
If the information requested
above is not provided, the CSSC may have difficulty in properly
following up with the team. Please be advised that the above
