Summer 2009 Exemption Form

If you captained a team in the sport in which you are registered for (Summer 2009) in 2008 or the Spring of 2009, you are exempt from having to go to the meeting. You must fill out the exemption form below or your team will receive a -1 spirit rating in the first game of the season, regardless of what you were rated by your opponent.

If you are exempting yourself from the Summer 2009 Player Meeting you are required to pick up your equipment. Pick-up information is provided below:

Dates:

Saturday July 11th, 2009 from 11:00am to 5:00pm

Location: Pick-up will be held at the CSSC Office.

Please make sure bring your Visa or Mastercard for the equipment deposit. Deposit costs are as follows:

Soccer (all divisions): $20
Kickball (all divisions): $40
Rec Slo-Pitch: $40
Rec Plus / Intermediate Slo-Pitch: $80

All players are required to bring their own shoes, bats, gloves and any protective wear that they deem necessary.

Please note: In filling out this exemption form, you are also agreeing to picking up your equipment on the date listed above.
